2.Proof of birth (1 is required)

• birth certificate or baptismal certificate  or passport

3.Immunization record with TB Risk Assessment (TDAP)

2.Prueba de Nacimiento ( se necesita 1)

3.Registro de vacunas con evaluación de riesgo de tuberculosis (TDAP)
